You will need a paper to protect the edges of your phone while you are removing the screen. Make sure you are not damaging the screen araound the edge while you lift up the damaged screen. Twist the screw driver to remove the screen without causing an to the phone.
Repeat the same step throughout the frames of the screen and slid a paper to the gaps where the screen was removed. After that just slid up the screen and remove it without disturbing the other parts of the phone.

Now you can install a new screen. Take the screen you had, make sure that it suits your phone model. Remove the coating of the new screen and remove dust and particles from the surface. Put a cello tape to that screen and carefully place it over your phone. After placing it correctly you can fix it. Just adjust the position and place it correctly and then use the knife to fix it tightly.
